 By submitting this form I allow Wesley Chapel's #1 Babysitting Services to take Pictures of   my child and i understand they could be used via advertisement on there business page or   website.

 

  I Agree i will not held Wesley Chapel's #1 Babysitting Services  accountable if for any reason we leave the agency and they continue to use those photos.

 

  i understand i have the right to not fill out this photo release form witch will with draw    my child from being posted for via advertisement.
